- Download the Form: Visit www.nhmharyana.gov.in to download the application format (p. 3).
- Prepare Your Documents: Attach self-attested photocopies of all educational certificates, experience certificates, residence proof, and caste certificates (if applicable) (p. 2).
- Submit: Send your completed application to the Office of DH&FW Society (Civil Surgeon Faridabad) before the deadline (p. 2).
- Verification: Carry your original documents for verification on the scheduled date and time at the Civil Surgeon's office (p. 2).
